When to Submit a Forms Ticket

Submit a ticket if you’re creating or changing something that requires backend adjustments or involves locked form types.

You’ll need to submit a ticket for:

  • Creating a completely new form from scratch

  • Editing the New Patient or Health History forms

  • Renaming a form (changing its title or name)

  • Modifying non-editable fields in an existing form

  • Changing a form’s category (e.g., Intake → Consent)

  • Updating the age threshold for minor forms (default: age 16)

Turnaround time: ⏰ Please allow up to 5 business days for completion after your ticket is submitted.

When You Can Edit Forms Yourself

If you’re making small content or layout updates in existing forms, you can make the changes yourself.

You can safely make these updates on your own:

  • Edit field labels or text

  • Change field verbiage (the wording in a question or paragraph)

  • Add new fields to an existing form

  • Mark fields as Required or Not Required

  • Hide or delete fields

  • Add placeholders in text

  • Add new pages within an existing form

How to Submit a Forms Ticket

  1. Click the question mark (?) icon located at the bottom of the left hand menu in Dental Intelligence

  2. Select Contact Technical Support

  3. Choose Create/Amend Form or Consent from the ticket list

  4. Upload your form file (.doc or .docx)

  5. Include any notes or special instructions
