Why We Need Your EIN Documentation for Toll-Free Number Verification

Learn what steps we'll need to take to resolve an EIN error so that you can begin using your toll-free number in Dental Intelligence

If your office is attempting to text patients from Dental Intelligence and it triggers Error 1112 in our system, we may ask you to share a copy of an IRS EIN confirmation document so that we can submit an appeal to the carrier on your behalf so that your toll-free number can be approved.

We know paperwork is never fun, so here’s a clear explanation of why this is required and how it helps complete the toll‑free texting approval process.

What is the EIN error?

In the internal systems-side of Dental Intelligence, we'll be notified that your texting request was flagged with the following carrier error:

Error 1112 – Business Registration Number (EIN) is missing or invalid.

This message comes directly from the carrier during toll‑free number verification.

What does Error 1112 mean?

This error means the toll‑free number cannot be approved for texting yet because the EIN and legal business details submitted don’t exactly match IRS records.

Common causes include:

  • A mismatch between the legal business name and the EIN

  • Formatting differences (for example, missing or incorrect hyphens in the EIN)

  • An outdated or incorrect business address

When this happens, the carrier requires an appeal before initial approval can be completed.

Why do you need documentation from us?

To submit the appeal on your behalf, the carrier must confirm your EIN directly against official IRS records. The only way they can do that is by reviewing an IRS‑issued confirmation document.

This step is required by the carrier and applies to all providers, not just our platform.

What document do I need to provide?

Please send Dental Intelligence one of the following IRS documents:

  • CP 575 – Original EIN Assignment Letter (likely something you have on file already), or

  • IRS 147C – EIN Confirmation or Replacement Letter

Either document works. These documents are used only to verify your EIN for the appeal and are not stored or used by Dental Intelligence for any other purpose.

Need help finding your EIN letter?

If you don’t have a copy readily available, the IRS can reissue an EIN confirmation letter (Form 147C).

You can contact the IRS Business & Specialty Tax Line at 1‑800‑829‑4933 during posted business hours to request a mailed replacement letter.

What information must be visible on the document?

To avoid delays, please make sure the document clearly shows:

  • Legal business name

  • EIN (with correct hyphen formatting, for example: 12‑3456789)

  • Business address

  • Entity type (if listed, such as LLC or Corporation)

What happens after I send the document?

Once Dental Intelligence has received your document:

  1. We submit the appeal to the carrier on your behalf.

  2. The carrier reviews and verifies the EIN against IRS records.

  3. If approved, the toll‑free number can proceed through the texting approval process.

Important: Until the documentation is received and the appeal is approved, the toll‑free number cannot be approved for texting.
