Porting Your Phone Number to DI Voice: What You'll Need to Provide

If you're moving your practice's existing phone number to DI Voice, we need some information from your current phone provider (the "losing provider"). Collecting it early helps avoid delays, because we can't get some of these details from anywhere else.

Note: This article covers porting your number for the DI Voice phone system functionality. If you're porting a number only for texting purposes and not using DI Voice as your VoIP provider, see this article.

What is a Customer Service Record (CSR)?

A Customer Service Record, or CSR, is a document your current phone or VoIP provider keeps on file for your account. It lists the account details tied to your phone number, and the new provider uses it to verify that you own the number and are authorized to move it.

How to get one: Contact your current phone or VoIP provider and ask them for a "CSR." If a CSR isn't available, a recent copy of your phone bill can often work as a substitute. Use whichever is easier to get.

Information we need

Please make sure the following information is included on your CSR or bill:

About the Port Out PIN

The Port Out PIN can only be provided by your current (losing) provider. We can't generate or look it up for you, so be sure to request it when you ask for your CSR. Some providers list it on the CSR, while others require a separate request.

What you don't need to worry about

Some details, such as your billing address, are collected by Dental Intelligence during account setup, so you won't need to provide them separately.

Tips for a smooth port

  • Request your CSR as early as possible. Your current provider controls how fast you receive it.

  • Check that the details are accurate. The account name, service address, and authorized person must match the provider's records exactly.

  • List every number you want to move. Confirm that all numbers appear on the CSR.

  • Don't cancel your current service yet. Canceling before the port is complete can cause you to lose your number.
