Automated Forms through Modento will save you and your team time & energy! Here is a short video going over the Forms section of the Modento Settings:

Step by Step Instructions:

  1. Go to Settings

  2. Select Forms

  3. Click the drop down menu in the Existing Patients column

  4. Select the availability rules for the Form

  5. These changes will save automatically, so no further action is required

These forms will be requested when the patient receives their Save The Date, Appointment Reminder, and Check-In Request messages which are found in the Appointment Communication section.
