When forms and consents are completed in Engagement, they will automatically write back into your practice management software. Depending on the type of software your practice utilizes, forms and consents will writeback into certain areas within your practice management software.
Important note: If you customize any of your forms, they will not integrate with your PMS the same way our standard forms do. As a result, certain information will not automatically sync or write back. Consider this before making any customizations. Learn more here.

Eaglesoft
Forms and consents will upload in SmartDocs in Eaglesoft.

How Forms Are Saved to Eaglesoft SmartDocs
Patients can conveniently complete their forms on their smartphone, tablet, or computer—whichever works best for them. Once the patient has filled out and signed their forms, your team will receive a real-time notification in the Engagement Personal Assistant. From there, you can open the patient chart in Eaglesoft and instantly see the form uploaded into SmartDocs. The uploaded PDF copy in SmartDocs includes:
The date the form was completed
An exact copy of the completed form
Signatures and timestamps
Any relevant consents
Engagement doesn’t just save PDFs — it also writes key patient information directly into Eaglesoft. This includes contact details and demographic information (written back directly into corresponding Eaglesoft fields).
Forms are automatically grouped into dedicated folders within Eaglesoft, so it’s always easy to find the documents you need. This seamless process applies to treatment plans, consent forms, and intake paperwork, ensuring all documentation is organized and accessible.
Note: Patient profile pictures submitted with Engagement forms are not written back into Eaglesoft. Patient profile pictures in Eaglesoft are not pulled into Engagement.
When patients complete a health history form, the process works the same way. Along with the PDF copy saved in SmartDocs, Engagement also writes back:
Medical conditions
Allergies
Medications
All of this patient health data is seamlessly inserted into the appropriate fields within Eaglesoft, eliminating the need for manual uploads by your team. Click here to learn more about mapping medical alerts.
For added transparency, all patient interactions — including sent forms, completed paperwork, and communication history — are automatically recorded in Eaglesoft’s note history section, giving you a complete, time-stamped record of communications between your staff and each patient.
Open Dental

Engagement automatically updates the patient’s medical conditions, allergies, and medications directly in Open Dental. Important note for Dentrix users: Dentrix version 23.3 is not compatible with forms writeback in Engagement. In order to writeback forms, you must be on Dentrix version 23.4.

Patients can conveniently complete their forms on their smartphone, tablet, or computer—whichever works best for them. Once the patient has filled out and signed their forms, your team will receive a real-time notification in the Engagement Personal Assistant. Then, the completed form is automatically saved directly into the patient Document Center within Dentrix. The uploaded PDF copy in Dentrix includes:
The date the form was completed
An exact copy of the completed form
Signatures and timestamps
Any relevant consents
Engagement doesn’t just save PDFs — it also writes key patient information directly into Dentrix. This includes:
Contact details and demographic information (written back directly into corresponding Dentrix fields)
Note: Patient profile pictures submitted with Engagement forms are not uploaded to the patient card in Dentrix; the photo is included within the form PDF that is saved. Patient profile pictures in Dentrix are not pulled into Engagement.
Completed forms are neatly organized into designated folders within Dentrix, so your team can quickly find the right documents when they need them.
When forms and consents are pushed into the Document Center, they will appear with a string of characters in the title/name. This behavior is expected. It prevents duplicate forms from being uploaded into your Document Center and cluttering your folders.

Important note for Dentrix users: In order for new patient forms to be automatically added into the Dentrix Document Center, they must have a patient chart created prior to submitting any forms.
When patients complete a health history form, the process works just as smoothly. The uploaded PDF copy in Dentrix Ascend includes:
The date the form was completed
An exact copy of the completed form

Engagement doesn’t just save PDFs — it also writes key patient information directly into Dentrix Ascend. In Dentrix Ascend, click on the patient to open their chart and then click Patient Information from the Patient menu, where you'll find:
Contact details and demographic information (written back directly into corresponding Dentrix Ascend fields)
Patient profile pictures (Note: Patient photos added in Dentrix Ascend do not pull into Engagement)
Click on Medical Alerts to see all mapped medical alerts (e.g., high blood pressure, diabetes) into Ascend's alert system (Note: Does not include any unmapped or custom-alert questions).
Note: Because Dentrix Ascend does not have an Emergency Contact field in the Patient Information section, emergency contact information captured in Engagement forms will not write back to Ascend but can still be located in the PDF copy of the form saved to the patient file.

Denticon
Forms and consents write back to the Notes section of the patient chart in Denticon.
Patient Information: What Writes Back to Denticon Patient Fields
Denticon has strict controls over which patient demographic fields can be modified through third-party integrations. Understanding these limitations is important for managing patient expectations and your workflow.

Important: If a patient attempts to update a protected field through a form, the change will appear in Dental Intelligence temporarily but will never transfer to Denticon. The field will revert to the Denticon value during the next data sync.
How Forms Are Saved to Patient Notes in Denticon
When a patient completes a form through Dental Intelligence, two things happen:
The completed form PDF is immediately written back to the Notes section of the patient's chart in Denticon (no approval needed)
Any patient information updates are queued for office review and approval before being applied to Denticon
This two-tier approach ensures you have complete control over patient data while still benefiting from automated form processing.
Important note about Responsible Parties: Forms can only be written back for patients, not for Responsible Parties. If you need documentation for a Responsible Party, you'll need to manually save or attach it in Denticon.
How to Accept Updates in Denticon
In Denticon, navigate to Patient > Online Registered Patients - Existing
You'll see a list of patients with pending information updates
Click on a patient to open their Patient Overview page
Look for the red bell icon — this indicates pending changes
Click the red bell icon to review the specific changes
Accept or reject the changes as appropriate
Important note: If you resync a patient before accepting their updates, the Dental Intelligence patient card will revert to match Denticon's data. The updates aren't lost — they're still in the approval queue — but the patient card will show the old information until you accept the changes.
Managing Medical Alerts from Forms in Denticon
Patients can update their medical history and health alerts through forms. Like demographic updates, these changes require office approval before they're applied to the patient's medical record in Denticon.
Critical Configuration Requirement
IMPORTANT: In Denticon, no medical alerts can have the "Set State as Required" option enabled. If any alerts have this setting enabled, the forms integration will not function properly for medical alerts.
Before deploying medical history forms, verify this setting for all your medical alerts in Denticon.
Reviewing and Accepting Medical Alert Updates in Denticon
Navigate to Patient > Online Registered Patients - Existing
Find the patient with pending medical alert updates
Open the Patient Overview page
Click on Medical Alerts
Click "Import Online Medical History"
Review the updates and accept them to add to the patient's medical record
