Using our standard forms in Engagement offers the most streamlined and professional experience for your practice. Our forms are carefully designed to integrate seamlessly with your Practice Management Software (PMS), ensuring that essential patient information is automatically updated, and health alerts are properly mapped.

In contrast, customized forms come with limitations, such as lack of PMS writeback, potential errors, and formatting issues, which can affect both efficiency and appearance. By using our standard forms, you’ll save time, reduce errors, and maintain a polished, professional look for your patients.


Benefits of Using Our Standard Forms

  • Seamless Integration: Our New Patient and Contact Update forms automatically sync with your Practice Management Software (PMS) when appropriate, updating key information such as name, address, email, phone number, and other essential patient details.

  • Designed for Your Practice: Our Health History form is specifically designed to gather the relevant information your practice needs, efficiently mapping health history data to alerts within your PMS.

  • Professional Quality: Our forms are meticulously reviewed for accuracy, ensuring they are free of typos and grammar issues, with a professional appearance throughout.

  • No Health History Mapping: Custom Health History forms do not support writeback or mapping of important health alerts (e.g., medications, conditions, allergies), which could result in missed alerts for your practice.

  • Formatting Issues: Custom forms may require manual copying and pasting, which can result in formatting issues that affect the overall aesthetic and professionalism compared to our standardized forms.
